COMMUNITY TRANSITION SERVICES
This service is designed to assist former residents as they make the transition back into the community. After a family leaves the residential program, they are contacted at intervals of one, three, six, nine, and twelve months. Our community transition counselor assists the family with clothing, school supplies, and referrals to community agencies, as well as employment, training, and housing searches if needed.
Topics such as nutrition, children's school attendance, finances, and other personal and/or social issues that affect self-sufficiency are discussed. The counselor provides Family Gateway program resources to the family in an effort to ensure a successful transition.
